Default Overheating in 1996 LS 400

I've had my radiator, thermostat, and several hoses replaced. The car worked fine after that. Then a few weeks later it started to overheat again. I noticed that coolant /anti-freeze is leaking somewhere. There are stains from the leaking or evaporation all around the reservoir. Any ideas?
